Before calling to schedule your initial evaluation we have provided a list of the information you will need, please read the coverage or payment option that applies below. Once you have all that you need please give us a call and we will be glad to schedule your initial evaluation with one of our Physical Therapists. If you are not sure what your coverage options are please just give us a call and we would be glad to help you.
Medicare and Private Insurance
- Before calling to schedule please have your insurance card ready.
- Also, have your referring physician information on hand.
- Please be prepared to answer short demographic information.
- When coming for the first time please bring your insurance card, prescription, and please arrive early to fill out a few short forms.
- We accept most major credit cards as well as most payments from health savings accounts (HSA.)
- Prescription for Physical Therapy is not required for evaluation (but preferred); but will be required for ongoing care.
Workers’ Compensation
- Before calling to schedule please have your claims adjuster information.
- We must have written authorization before we can schedule your evaluation.
- Also have your referring physician information on hand.
- Please be prepared to answer short demographic information.

Self-Pay
- The Body Center offers competitive rates for self pay patients.
- Please call for current fees for evaluations and follow up visits as well as custom orthotics.
